MSHSL Sports-Qualifying Physical

The Minnesota State High School League physical form is required to be on file with the school district prior to registration.

A student's physical form is valid for three years from the date of exam.

Submitting the Physical Form
For Fall Dance, Mountain Biking, Spartan Running, Tennis - Boys & Girls (7-8), Volleyball (6-8), Golf (7-8) and Track & Field (6-8), submit your physical form to Community Education:
  • Scan and email it to kim.king@orono.k12.mn.us. Either a very clear photo or use a scanner app on your phone to send a clear image.
  • Fax it to 952-449-8359
  • Drop it off at the Orono Activities Center front desk to the attention of Kim King.
For Cross Country Running (7-12), Swim & Dive - Girls (7-12), Winter Dance Team (7-12), Wrestling (7-12), Boys Volleyball (7-12), and all other high school athletics, submit your physical form to the High School Athletics office:
